Roche received complaints about the frequent occurrence of samples being flagged with [Clot.?] or [<CTval] with different assays. Investigations have confirmed that using a cobas t 511 / t 711 coagulation analyzer together with the Greiner VACUETTE® TUBE 3.5 ml for closed tube sampling, that has a high sample fill level can cause the system to fail in correct liquid level detection. Under wet conditions, provoked by the high sample fill level with this tube type, a continuous liquid film can form that bridges the stopper's unique recess directly to the sample surface. Consequently, the pipetting is executed in the stopper area. In rare cases, this can result in wrong, unflagged assay results for aPTT or D-Dimer or wrong HIL Test results. No harm to patients or adverse events have been reported regarding this issue.
